How do you change a culture?

How can you change the set of behaviors and habits that a couple, a family, or an organization has developed over time?  How do you shift the mindset that thinks, “This is who we are and what we do,” when sticking with the old way of thinking won’t produce the new results you want?  I’m sure something like this helps but in truth it’s not that simple.  And I need more than just this too – as familiar as these opening words might be for some people.

When you know things aren’t working, or at least that things aren’t heading in the direction you want to go, how can you be an effective change agent?
